All forms used in sex offender/child kidnapper registration have been revised. Previous versions of these forms will not be accepted after December 31, 2008.

The revised forms are listed below.

Use this form if:

• You are registering for the first time in Alaska;
• You are re-registering in Alaska after release from incarceration or upon return from outside Alaska; or
• You are completing your annual or quarterly written verification of your registration information.

Use this form if:

• You are registering for the first time in Alaska; or
• You are re-registering in Alaska after release from incarceration or upon from outside Alaska.

Use this form if you need to submit proof of your unconditional discharge for the sex offense or child kidnapping requiring registration.

Use this form to report changes to your registration information between scheduled written verfications.

Use this form if you are no longer physically present in Alaska and wish to have your registration information removed from the Alaska Sex Offender/Child Kidnapper Central Registry. Your information may not be removed until you have submitted this completed form and the department has verified its accuracy and completeness.
